Set Clear Goals for Your Benefits Survey

Explain the purpose in the introduction: We are reviewing our benefits and would like your feedback on priorities and ease of use. Please do not include medical information or details that identify another person. Results will be considered alongside the organization's budget and available plan options.

Questions About Coverage Priorities

Ask: Which three benefit categories are most important to you? How well does the current package meet your needs, from 1 to 5? Which one area would you most like us to review? Include options such as health, dental, income protection, mental health support and spending flexibility, with a prefer-not-to-answer choice.

Questions About Access and Communication

Ask: Do you know where to find your benefit details? How easy is it to understand what is covered? Do you know who to contact about a claim or enrolment issue? What general improvement would make the plan easier to use? Remind respondents not to name a diagnosis or describe a personal claim.

Protect Privacy in Small Teams

Avoid collecting names or detailed demographic combinations unless there is a clear, reviewed purpose. Small-group results can identify individuals even without names. Explain how responses are handled and who sees them; do not promise anonymity if the survey settings or process cannot deliver it.

Turn Feedback Into Plan Decisions

Group responses by theme, distinguish access problems from coverage gaps and compare priorities with actual plan options. Tell employees what was learned and what will be considered. A survey is input to a decision, not a promise to add every requested benefit.
